We’re happy to announce that we’ve opened medical services to all eligible American Indian/Alaska Natives living in Pierce County.
Enrollment is dependent on your current patient status with PTHA. Once enrolled, a Patient Access Representative (PAR) will assist the patient in choosing a Primary Care Provider. Please keep in mind that it may take some time to establish primary care, but same-day services will still be available for more immediate care needs.
Never Been a Patient with PTHA
To register for services, please call us and a Patient Access Representative to follow up with your registration.
- Tribal enrollment card, letter of descendancy, or Certificate of Indian Blood (CIB or CDIB)
- Proof of residency for Pierce County
- Birth certificate (or hospital announcement) for newborns
- Marriage/divorce documents to reflect name changes from Birth Certificate
- Current insurance information, if any (Medicaid, Medicare, or Private Insurance
Currently a Patient Utilizing Other Services
Patients who are currently registered and up to date for non-medical services at PTHA will immediately be eligible to enroll for medical services. To enroll and schedule an appointment with a Primary Care Provider, please call (253) 593-0232.
Wanting to Re-establish/Not Updated Within One Year
Complete Annual Updating:
- Verify your information with PTHA staff (phone, address, etc.) or update through patient portal (patient registration).
- Submit proof of Pierce County residency.
- Complete Washington State Apple Health or DSHS application process (if applicable).
Once annual updating is complete, you may schedule an appointment with a Primary Care Provider by calling us or using the Patient Portal.
